1-install: add startup.nsh if it doesn't exist

On some buggy UEFI firmware (e.g., VirtualBox), the boot entry created
by efibootmgr may disappear after poweroff. As a result it can't boot
to Android-x86 anymore.

Add startup.nsh to run our boot loader as a fallback.
