DSDA: Skip over held call when adding 2nd call

- When user attempts 2nd call over a different sub, if the existing call
  is already HOLDING, do not attempt to hold it again.
- Cause of bug: attempting to hold an non-ACTIVE call may time out, and in
  turn terminate the 2nd call as well.
