Finalize Multiendpoint functionality. 1) Map the "maximum calls exceeded" disconnect cause. 2) Fix bug where ImsConferenceController would consider external calls as potentially conferenceable. 3) Map through the PULLING state. Change-Id: I2920ac067ad5be35f6a9158d0f0e514721a8b4f6
